Human factors and quality control procedures: An example from the offshore oil & gas industry

Abstract

Major industrial accidents often evolve through cascading mechanisms comprising human factors, technology and organizational issues. To minimise their occurrence, practitioners share knowledge worldwide through international codes and standards, while governments and regulators are compelled to strengthen regulatory requirements. Based on these codes, standards and regulations, among other inputs, organizations are required to establish quality control procedures in order to ensure that the maintenance work is capable of sustaining equipment and systems’ performance during the facility’s lifecycle. This study compares a major industrial accidents dataset with data extracted from onboard safety audits conducted by the Brazilian oil and gas regulator. In this context, this paper aims to analyse quality control procedures applied by approximately 30 companies operating offshore in Brazil and to reveal how human errors are involved and addressed by this critical control. Further open challenges are then discussed.
